Accident Summary:

On mission escorting personnel detector aircraft which reported a hot area. As lead gunship started suppressive fire, the pilot felt a severe jolt on his right foot. The right door gunner's protective rod on his M-60 failed allowing the gunner to inadvertently hit the right side of the aircraft. The safety rod was attached to the barrel of the M-60 machine gun with safety wire instead of bolted securely in place.

The 119th AHC unit history records: While flying in the vicinity of FSB BLACKHAWK, 25 Km east of Pleiku on highway 19, CROC 462 was enroute to fly highway cover when they came under SA fire. Two rounds penetrated the fuel cell and the pilots made a precautionary landing. There were no injuries to the crew
