Accident Summary:
The aircraft was enroute from Phu Heip to An Khe. There had been no unusual occurences on take off
and the aircraft was five minutes north of Tuy Hoa at 4000 feet. The aircraft commander had transferred
the controls to the copilot. The engine tachometer went to zero about two minutes later. The aircraft
commander checked his instruments and found them to be in the green with gas producer at 91%. He assumed
the engine tachometer had failed. Fifteen seconds later the engine quit altogether. The engine
stoppage\was due to a break in the fuel line, thus causing fuel starvation. Failure of the engine
tachometer was loss of fuel which caused the engine to cease operation. The aircraft commander entered
autorotation holding 65 knots indicated air speed. He made a may day call and attempted an engine restart.
The N1 reached 35%, he released the trigger, but the N1 dropped to zero again. At this time, he was
approximately 1500 feet above the ground and close to the spot he had picked for a landing. Thinking
he was going to overshoot, he zeroed out his airspeed and again set up an autorotation using 65 knots
IAS and approximately 310 RPM on the rotor. He began his flare at 70 feet above the ground. The pilot
pulled initial pitch at approximately 10 feet and started to settle. At this point the aircraft had
little forward airspeed. He further stated that at approximately 3 feet the aircraft fell through
because he had pulled pitch too soon. The aircraft hit hard, folding back the skids, and causing major
damage to the aircraft. Skid marks on the ground indicated the aircraft slid 5 to 7 feet
