Accident Summary:

Aircraft UH-1D 65-9904 was sitting on POL pad running when UH-1H 66-16284 came to hover and made pedal turn to right causing its tail rotor to contact the UH-1H main rotor. The 90 degree gear box separated from the UH-1H and it turned 180 degrees to the right. Both skids and cross tubes collapsed.
