Accident Summary:
AT 0900 HOURS, 8 OCTOBER 1970, AIRCRAFT UH-1H, SERIAL NUMBER
66-16305, WITH WO1 BEVERLY AS AC AND WO1 WILLIAMS AS PILOT,
EXPERIENCED ENGINE FAILURE. THE ACCIDENT OCCURRED WHILE ON
TAKE-OFF FROM NAM CAM HELIPAD REPUBLIC OF VIETNAM, COORINATES VQ
9967. AT THE TIME THE AIRCRAFT WAS CHALK TWO IN A FLIGHT OF TWO
ON TAKE-OFF. THE AIRCRAFT WAS AT APPROXIMATELY 50 FEET AND 30
KNOTS. THE AIRCRAFT HAD FOUR NAVY SEALS ON BOARD AND WO1 BEVERLY
WAS AT THE CONTROLS. BOTH PILOTS WERE ALERTED OF THE EMERGENCY
WHEN THEY HEARD A LOUD EXPLOSION FROM THE ENGINE. BEING ONLY AT
50 FEET OF ALTITUDE WO1 BEVERLY DID NOT HAVE TIME TO CHECK HIS
INSTRUMENTS PRIOR TO GROUND CONTACT. ANALYZING THE MALFUNCTION
AS ENGINE FAILURE, WO1 BEVERLY IMMEDIATELY FLARED THE AIRCRAFT TO
REDUCE FORWARD AIRSPEED. THE AUTOROTATION WAS A STRAIGHT IN AS
THE WIND WAS CALM AND NOT A CONTRIBUTING FACTOR. THE ONLY
TERRAIN WO1 BEVERLY HAD TO LAND IN, CONSIDERING HIS ALTITUDE AT
THE TIME, WAS LESS THAN DESIRABLE. IT WAS COMPOSED OF UNGRADED
SOFT SAND THAT WAS BEING USED IN CONSTRUCTION OF A NEW RUNWAY IN
THE VICINITY OF THE HELIPAD. AS WO1 BEVERLY BEGAN TO LEVEL THE
AIRCRAFT THE REAR AREA OF HIS SKIDS SUNK IN THE SOFT SAND CAUSING
THE AIRCRAFT TO MOMENTARILY REMAIN IN A NOSE HIGH ATTITUDE. THE
RESULT OF THIS WAS A TAIL LOW ATTITUDE CAUSING A TAIL ROTOR
STRIKE AND SUBSEQUENT SEPARATION OF THE TAIL ROTOR BLADES FROM
THE HUB ASSEMBLY. THE TAIL ROTOR STRIKE, ALONG WITH THE SLIGHT
FORWARD MOVEMENT OF THE HELICOPTER, CAUSED THE AIRCRAFT TO SLIDE
FORWARD APPROXIMATELY 100FEET. DURING THIS GROUND SLIDE THE
CROSSTUBES COLLAPSED CAUSING THE AIRCRAFT TO COME TO A STOP ON
ITS BELLY. THE CREW AND PASSENGERS IMMEDIATELY EXITED THE
AIRCRAFT AS SOON AS IT SLID TO A STOP. THE AIRCRAFT WAS RETURNED
TO CAN THO ABOUT 6 HOURS LATER.\\
