Accident Summary:
THE CREW OF 66-16464, HAVING OFF LOADED WOUNDED PERSONNEL AT THE
ARVN MED PAD CALLED SOC TRANG TOWER FOR DEPARTURE INSTRUCTIONS.
THE ARVN MED PAD IS LOCATED APPROXIMATELY 300 METERS TO THE
NORTHEAST OF SOC TRANG ARMY AIRFIELD PERIMETER ON AN ABANDON ROAD
RUNNING EAST AWAY FROM THE AIRFIELD. THE TERRAIN ON EITHER SIDE
OF THE ROAD BEING UNTILLED RICE PADDIES AND ARE THREE FEET LOWER
THAN THE ROAD. THE PAD ITSELF IS OF PERFORATED STEEL PLANKING
(PSP) WITH TYPICAL WHITE H PAINTED ON IT. THE PSP WAS ATTACHED
TO THE GROUND WITH TWO INCH PIPE WITH FLANGED EDGES DUE TO BEING
DRIVEN IN THE GROUND. THE PIPES EXTENDED FOUR TO SIX INCHES
ABOVE THE STEEL PLANKING. AS THE PILOT STARTED TO LIFT THE
AIRCRAFT FROM THE PAD IT TILTED BACK AND TURNED ON ITS RIGHT
SIDE. THE RED MAIN ROTOR BLADE STRUCK THE TAIL BOOM AT STATION
410 JUST FORWARD OF THE 45 DEG GEAR BOX SEVERING THE TAIL BOOM.
THE MAIN ROTOR BLADE ALSO STRUCK THE TAIL ROTOR. THE 90 DEG GEAR
BOX WAS SEPARATED FROM THE VERTICAL FIN AND WAS LOCATED 15 FEET
TO THE REAR OF THE AIRCRAFT. THE TRANSMISSION AND PYLON ASSEMBLY
WAS ALSO SEPARATED AND WAS 55 FEET AWAY ON A HEADING OF 150 DEG.
THE HEAD ASSEMBLY AND BLADES WERE LOCATED 8 FEET FROM THE TOP OF
THE AIRCRAFT. ALL AIRCRAFT PARTS AND COMPONENTS WERE LOCATED
WITHIN 30 METER RADIUS OF THE MED PAD AND/OR IMPACT AREA.\\
