Accident Summary:
THE MISSION FOR AIRCRAFT NUMBER 67-17683 WAS A 90 DAY
STANDARDIZATION RIDE FOR THE PILOT, WO1 LUCKY H. WILSON. 1LT
GEORGE JAMESON WAS THE INSTRUCTOR PILOT. TOTAL FLIGHT TIME FOR
THE DAY AT THE TIME OF THE ACCIDENT WAS APPROXIMATELY 04+20. THE
TRAINING FLIGHT WAS IN PROGRESS FOR 01+15 PRIOR TO THE ACCIDENT.
THE AIRCRAFT HAD BEEN REFUELED PRIOR TO TAKEOFF AT PHU HIEP WHICH
WAS AT APPROXIMATELY 1345. THE CREW HAD BEEN PRACTICING
AUTOROTATIONS TO RUNWAY 21R AT TUY HOA. THEY WERE TOLD BY THE
TOWER TO HOLD NORTH OF THE AIR BASE UNTIL THE LANDING FIXED WING
AIRCRAFT HAD CLEARED THE ACTIVE RUNWAY. DURING A PREVIOUS HOUR OF
INSTRUCTION, THEY HAD HELD NORTH FOR A CONSIDERABLE AMOUNT OF
TIME. IN ORDER NOT TO WASTE THIS TIME HOLDING, THE INSTRUCTOR
PILOT ELECTED TO DEMONSTRATE A SIMULATED ANTI-TORQUE FAILURE
USING A SMOOTH SAND BAR ON THE SOUTH SIDE OF THE DA RANG RIVER
ABOUT 1 NM NORTH OF TUY HOA. THE WINDS WERE 110 - 130 AT 8 KNOTS;
THE DIRECTION OF THE FINAL APPROACH WAS ABOUT 210 DEG. AS HE
APPROACHED THE TERMINATION OF THIS MANEUVER THE IP SLOWED HIS
AIRSPEED AND ANTICIPATED THE SLOW SWING TO THE RIGHT OF
AIRCRAFT'S NOSE. AS THE NOSE CAME TO THE RIGHT BUT STILL LEFT OF
CENTER AND WITH THE AIRCRAFT JUST A FEW FEET ABOVE THE SAND, THE
IP ATTEMPTED TO INITIATE A GO-AROUND. AS HE DID THIS, THE SKIDS
MADE CONTACT WITH SAND, SLID FOR A DISTANCE BURYING THE RIGHT
SKID AND THE AIRCRAFT ROLLED OVER. THE MAIN ROTOR AND THE
TRANSMISSION WERE TORN FROM THE AIRCRAFT, ALONG WITH THE TAIL
ROTOR AND THE 90 DEG GEAR BOX. THE AIRCRAFT FINALLY CAME TO
REST, INVERTED, APPROXIMATELY 15 METERS FROM FIRST POINT OF
CONTACT. BOTH PILOTS WERE UNINJURED AND EXTRACTED THEMSELVES
FROM THE WRECKAGE AFTER TURNING OFF THE FUEL AND BATTERY. THERE
WAS NO POST CRASH FIRE.
